Getting Started
My Program For Life is a program to help you succeed on your weight-loss journey. Below are the steps you will need to take in order to prepare for your surgery and achieve optimal long-term success:
Step 1
Attend Free Weight loss Surgery Patient Seminar
In order to make an informed decision, your 1st step should be to attend a Free My Program for LIFE informational seminar. At the end of the seminar you will be able to schedule a consultation with the Bariatric Surgeon of your choice.
- Meet the surgeons and ask questions
- Hear stories from patients who have already undergone bariatric surgery
- Learn how to obtain authorization from your insurance company.
- Determine if weight loss surgery is the right choice for you?
While attending a Free My Program for LIFE educational seminar you will receive all of the information needed to get you on the right track for your surgery.
Step 2
Call insurance company
How do you get insurance to pay for your weight loss surgery?
Our My Program For Life team of insurance specialists will answer all of your questions and provide detailed, proven instructions on how to get insurance approval at our Free Weight Loss Surgery Seminar.

Step 3
Office Visit
At the end of the seminar you will be able to schedule a consultation with the Bariatric Surgeon of your choice.

Education

- National Institutes of Health (NIH)
Step 5
Medical Evaluation
In order to proceed for surgery you may need all or some of the evaluations below:
- Psychological Evaluation
- Nutritional Evaluation
- 6 Months Notes (your insurance may require 6 months of participation in a medically supervised weight loss management program in order to obtain approval)
- Primary Care Physician Letter of Medical Necessity
- Blood Work (you will receive a script from your surgeon at the seminar in order to get this done)
- Clearances
- Cardiologist
- Gynecologist
- Endocrinologist
- Primary Care Physician
- Hematologist
- Gastroenterologist
- Pulmonary
- Possible other clearances may be needed (This will be discussed with you individually)
Step 6
Final Visit Before Surgery
- You will have the ability to answer any and all last minute questions.
- You will receive your post-operative diet plan
- You will schedule you post-operative appointment with your surgeon
